Release Medical Records

In order for Dr. Lowery to receive a copy of your past medical history, his office needs a copy of your medical chart from your current/previous physician. All new patients must contact that physician office and submit a Medical Chart Release Authorization Form to them. Many offices utilize patient portals for you to complete this electronically. If you have trouble getting your records, please send this form to us and we will submit it on your behalf. This sometimes expedites the process.

A standard form for you to print, complete, and submit to your former physician is available here: 

**VILLAGE HEALTH PARTNER PATIENTS: VHP charges its patients a $25 fee (and utilizes a third-party software to collect that payment) for your records release. We have learned that your medical records may be available to you FREE OF CHARGE via your VHP Patient Portal. If you are able to download your records from the portal, simply print and bring to your next appointment or fax them to us at 469.287.7903.
